1. Who we are and what this covers

This patient platform (the "Platform") is operated by Meeta Singh MD PC ("we," "us," "the practice"). These Terms govern your access to and use of the Platform as a patient of the practice. By creating an account or using the Platform, you agree to these Terms.

The Platform is a supplemental tool, not your medical record. Your official medical record is maintained separately in the practice's electronic health record system. The Platform provides three things depending on your program: a daily journal, a view of your sleep-ring data trends, and — for Predictive Sleep Screening patients — access to your screening report once the physician has reviewed and released it.

2. The Platform is not for emergencies or urgent care

Journal entries and ring data are reviewed by the practice during the normal course of care, not continuously and not on a guaranteed timeline. Nothing you enter into the Platform should be relied upon to trigger an immediate clinical response.

3. Eligibility and your account

• You must be an active patient of the practice (or an authorized representative of one) to use the Platform.
• You must be 18 or older, or have an account established and supervised by a parent or legal guardian.
• You are responsible for keeping your login credentials confidential and for enabling the security features offered (including multi-factor authentication where available).
• You agree to notify the practice promptly if you suspect unauthorized access to your account.

4. Acceptable use

You agree to use the Platform only for your own care and only as intended. You will not:

• Attempt to access another patient's data or any account that is not yours.
• Upload malicious code, attempt to breach security, or interfere with the Platform's operation.
• Use the Platform to harass any person or to submit unlawful content.
• Reverse-engineer, scrape, or copy the Platform other than your own data.

5. The journal and other content you submit

You own what you write in your journal. By submitting entries, you grant the practice permission to access, store, and use them for your care and for the practice's normal operations as a healthcare provider. The practice does not use automated AI analysis on your journal entries — they are read as you wrote them. Journal entries are part of your information held by the practice and are handled under the Privacy Policy.

6. AI-assisted reports (Predictive Sleep Screening only)

For Predictive Sleep Screening patients, the practice uses AI tools to help draft your screening report. Every report is reviewed, edited as needed, and approved by Dr. Singh before it is released to you. No report reaches you without physician review. AI assistance does not replace the physician's clinical judgment, and the released report reflects the physician's professional conclusions. AI tools are not used to generate reports for the Insomnia Treatment or Sleep Apnea Treatment programs.

7. Ring data and third-party services

Sleep-ring data shown on the Platform originates from your monitoring device and a third-party monitoring service. The practice presents trends drawn from that data for your convenience. Data may be delayed, incomplete, or contain gaps (for example, nights the device was not worn). Trend views are informational and are interpreted by your physician in clinical context — do not self-diagnose or change treatment based on what you see in the Platform.

8. No guarantee of availability

We aim to keep the Platform available but do not guarantee uninterrupted access. We may modify, suspend, or discontinue features, perform maintenance, or update the Platform at any time. Your care does not depend on the Platform; if it is unavailable, contact the practice directly.

9. Intellectual property

The Platform, its design, and its software are owned by the practice or its licensors. These Terms grant you a limited, personal, non-transferable right to use the Platform for your care. They do not transfer any ownership in the Platform to you. Your own data remains yours.

10. Disclaimers

Except as required by law, the Platform is provided "as is." The practice does not warrant that the Platform will be error-free or that data displayed will always be accurate or complete. This section does not limit the practice's professional and legal obligations to you as your healthcare provider; it addresses only the software tool itself.

11. Limitation of liability

To the maximum extent permitted by law, the practice is not liable for indirect, incidental, or consequential damages arising from your use of the Platform as a software tool. Nothing in these Terms limits any liability that cannot be limited under applicable law, including liability arising from the provision of medical care.

12. Termination

You may stop using the Platform at any time. We may suspend or terminate your access if you violate these Terms or when you are no longer an active patient. Termination of Platform access does not affect your medical record, which the practice retains as required by law. On request, and consistent with the Privacy Policy, you may obtain a copy of your data.

13. Changes to these Terms

We may update these Terms. If we make material changes, we will notify you through the Platform or by the contact method on file and update the "Last updated" date. Continued use after changes take effect means you accept the updated Terms.

14. Governing law and disputes

These Terms are governed by the laws of the State of Michigan, without regard to conflict-of-law rules. Any dispute arising out of or relating to the Platform will be brought exclusively in the state or federal courts located in Michigan, and you consent to the personal jurisdiction of those courts. Nothing in this section limits any right or remedy that cannot be waived under applicable law.
